3. Medical records release

The federal Health Insurance Portability and Accountability Act of 1996, a.k.a. HIPAA and most state laws force all health providers to not share a patient’s medical information without a valid authorization. That’s why a medical information release must be at hand.

This is a document that entitles someone to see your medical records. Just like in the case of a health care proxy, the person authorized to get your medical records should be your spouse, as he or she knows best your previous and current health issues. However, you can name whoever you want.
